Do I need a referral from my physician prior to receiving physical therapy services?

In the state of Ohio, a physician referral is not required for physical therapy evaluation. However, the physical therapist shall inform the patient’s health care provider within five days of initial evaluation and must refer the patient back to the appropriate health care provider if no progress is made in 30 days or symptoms or conditions are beyond the scope of practice of the physical therapist. Can I be seen by a member of Longevity Brain and Body Wellness, LLC if I have Medicare primary insurance?

Currently, Longevity Brain and Body Wellness, LLC is not a participating provider with Medicare. We are unable to accept clients with Medicare as their primary insurance for speech and physical therapy services. However, clients with Medicare insurance may participate in brain health programs, preventative services, wellness consultations, and fitness programs on a private pay basis.

I made an appointment but need to change or reschedule my appointment. Will I be charged for this?

We request at least 24 hours advance notice for cancellation. Cancelled appointments less than 24 hours in advance may be charged in full. We do understand that extenuating circumstances occur and will make reasonable efforts to reschedule a cancelled appointment.

Where are services provided?

Longevity Brain and Body Wellness, LLC is currently a mobile business providing in-home services to our clients.

Can services be provided in a location other than my home?

Longevity Brain and Body Wellness, LLC may provide services outside of a client’s home on a case by case basis. For example, an ergonomic consultation may be completed at client’s workplace (with employer consent) or speech therapy and brain health services may be provided at a local library or public establishment pending the mutual agreement of client and therapist. Clients are responsible for ensuring permission to utilize locations outside of their home environment.

What areas does Longevity Brain and Body Wellness, LLC provide services?

Currently, we provide services to clients in Avon, Avon Lake, Bay Village, North Olmsted, Olmsted Falls, Rocky River, and Westlake. Service to clients outside of these cities may be provided on a case by case basis with additional travel fee at the client’s expense. Please contact us for further information.

How long is a typical evaluation/consultation and follow up visit?

Each individual session will typically last 60-75 minutes. Group consultations may vary.

Does Longevity Brain and Body Wellness, LLC provide telehealth services (i.e. evaluation or consultation via video conference or phone?)

Yes, we are currently offering virtual visits for all of our services at this time in response to Covid-19. We are able to provide virtual visits to Ohio residents only. Please contact us for further information.

How do I schedule a virtual visit?

To schedule a virtual visit, simply book an appointment on our website as before using Acuity scheduling. You will receive an email confirming your appointment, followed by an email from us with a meeting link for your virtual visit.

What technology do I need for a virtual visit?

You will need a computer or laptop with a working camera and audio, or a tablet or smart phone for the virtual visit. It is recommended you download the Google Meet app prior to your appointment.

What video conferencing service are you using for virtual visits?

We are using Google Meet.

Do I need a Google account to participate in a virtual visit using Google Meet?

No, you do not need a Google account to participate. You will simply click the meeting link in the email you receive from us at the time of your scheduled appointment. Once redirected to the Google Meet website, you will click "Ask to Join" and will be invited to join the virtual visit. Please note: if you are using a mobile device you are required to be signed into a Google or Gmail account to join.
